Preparing for Licensure
As your application is received, a licensor will be assigned who will be available for consultation and to assist you in the licensing process. The following are examples of documentation to prepare, and other agencies you may need to contact for approval of the program's physical facility:
- Program Administration requires information on your governing body, client records, employee records, written policies and procedures and organization charts, etc.
- Background Clearances are required for providers serving children or vulnerable adults. See our background-screening page to get started on background checks.
- Local Authorities may require building permits, business licenses, zoning approval, building code approval, fire department, and health department approval, etc. Ask your licensor for guidance in working with these agencies.
