Renewal Licenses

Renewal licenses are not available on-line yet. You will need to fill out a renewal application . Please do not submit your renewal information through our online submittal page, this could delay the processing of your renewal.

Mail renewal application and check to:
Office of Licensing
120 North 200 West, #303
Salt Lake City, Utah 84103.

You will be notified of an appointment for the required site visit by the licensor assigned.

For best results, apply and prepare for the renewal visit early. Expired licenses are not backdated. Visit our forms page for posters, forms, and information on renewal fees.
