Background Screening Application

The background screening application can be acquired from the Licensee Representative or downloaded here

Instructions for completing the application are located here

Remember these important points when filling out your application:
  Include full first, middle, maiden and previously married names
  Utah residents require copies of a Utah Driver License or State ID card
  List out of state/country with date information if applicable
  If convicted of a crime, the court docket showing final disposition must be provided.
  For hard-copy fingerprint cards the correct fee is exactly $30.25 and the payment is to be made to the Department of Public Safety.
  For electronically scanned fingerprint transmission the correct fee is exactly $34.25 and the payment is to be made to the Department of Human Services. The Office of Licensing will issue an Authorization letter and the list of scanning sites.
 
Here are some common reasons applications are returned:
  Application is incomplete or illegible.
  Copy of Driver License or State Identification Card not attached.
  Outdated request form (check the upper right hand corner to be sure you have the April 2007 or later revision date).
  Fee amount is wrong
  Cashier check or money order is made payable to the wrong agency.
